Tobacco-free Affidavit

The Tobacco-free Affidavit for 2014 will be available during Open Enrollment, Nov. 11 through 22. Employees must complete the affidavit for themselves and for their covered spouse online each year in order to receive the Tobacco-free premium reduction. Employees can sign and submit the affidavit in OneStart when selecting a medical plan. The employee's medical plan contribution will be reduced by $25 per month for an employee or spouse ($50 for both) who do not use tobacco.

IU High Deductible Health Plan (HDHP) PPO & Health Savings Account (HSA)

This Open Enrollment, consider the IU HDHP PPO & HSA, which has the lowest employee premium contribution of the four plan options. When enrolling in the HDHP PPO & HSA, IU contributes $1,250 (employee-only coverage) or $2,500 (all other coverage levels) to the HSA, which covers the deductible, should you choose. Preventive services and certain prescriptions have no deductible or co-insurance, and other preventive prescriptions have only a co-insurance. The HSA feature of this plan provides tax-free savings and earnings that can be used now, in the future, and even into retirement.

Contribution Amounts

Premium contributions are based on your annual base salary. This amount is located in the Employee Center in OneStart in the Total Compensation section, the top left option.

Benefit Briefs

Short, narrated benefit presentations are available online. They describe various features of the plans, such as how deductibles work, how to use the HSA, and information on preventive prescriptions and medical benefits.
